These four pictures below was a remodel I did about 15 years ago. The upper left shows the porch tear out. The upper right shows the poured form. The lower left shows the frame and the lower right shows the shear and the windows. Maybe someday I'll take a photo of the finished project and post it.

The bottom four photos show how a typical garage floor is built. Upper left is the trench dug to the right depth. Then the form is built. Then the cement truck is called and the pouring begins. The lower left shows the forms removed. The lower right is a photo of the right side of the addition.

The photo below shows the project near completion. In the upper right you can see where the new facia meets the old.
